The titular Omori serves as the final boss enemy in Omori for the Sunny Route, in which Sunny finally decides to face his guilt over Mari's death and try to move on from the past.
Story
After a disastrous encounter with Basil, Sunny finds himself in Headspace once more, where he navigates Memory Lane, remembering the context behind many of Basil's photos. Each memory repairs his shattered violin: After it is fully repaired, Sunny finds himself backstage, where his friends offer final words of encouragement before fading, leaving Sunny alone to play his part of the Duet. Shortly after he finishes it, he breaks down in tears, overwhelmed by memories and grief, and suddenly wakes up in White Space, as himself.
Sunny explores White Space, and quickly finds Omori waiting for him. The two promptly begin battling for the fate of Sunny's mind.
The battle consists of six phases, each phase ending after Omori's health is depleted. Omori will grow stronger with each phase, and the background and music will slowly distort into more disturbing forms. Omori cannot be defeated by conventional means and will eventually triumph over Sunny. However, after the very first time Sunny is made Afraid during the battle - the earliest being the beginning of the fifth phase - dying will instead prompt Sunny on whether or not he wants to Continue, instead of Retrying the battle. His decision will directly affect the game's ending:
- Good Ending: If the player chooses to continue, Sunny stands back up, and begins to play his Duet with Mari, as Sunny remembers the many childhood memories he shared with Mari and his friends before her accidental homicide. After the Duet concludes, Omori embraces Sunny, before dropping his knife and disappearing, seemingly absorbed into Sunny. Sunny will awake in his hospital bed, in tears as he finally comes to terms with Mari's death. He then seeks out Basil's room, where his friends are tending to a hospitalized Basil, and prepares to tell them the truth behind Mari's death and alleged suicide.
- Bad Ending: If the player chooses not to continue, Omori and Sunny will embrace; although this time, Sunny will instead drop his violin and disappear, as Omori presumably completely takes over his mind and body. Omori will reappear in White Space and will prepare to go on another adventure with his friends in Neighbor's Room, although his friends are eerily silent. As soon as Omori and his friends depart to Headspace, Sunny will appear on the hospital's balcony. He ultimately decides to commit suicide by throwing himself off the hospital roof.
Battle
Omori serves as the final boss of the Sunny Route, serving as the final obstacle Sunny must face before he is willing to face the truth of the circumstances regarding Mari's death and face his friends. The battle is divided into six phases, and he behaves as follows:

Trivia
- Even if cheats are used to defeat Omori's sixth phase, the sixth phase will simply repeat indefinitely until Sunny falls.
- The player has a natural 75% chance of hitting right in the heart, while Omori can only hit in the heart under two circumstances: Using Stab for a guaranteed one, or the Red Hands he uses on the second phase, which have a 10% chance of hitting in the heart per hit.
- Stab and Hack Away are modified versions of the attacks he used on his Headspace adventures. He also notably uses Red Hands, an attack that can only be obtained on the Omori route.
- His real stats are 80 , 4 attack, 2 defense, 6 speed, and 10 luck; however, right after the battle begins, an event is called that will significantly raise his stats.
- Interestingly, derelict code relating to the Omori fight suggests that Omori's strength in the battle would've been directly affected by the level he was at while the player was controlling him. In practice, this code never executes.
- Contrary to popular belief, Omori's raw stats do not increase as the fight goes on. Rather, his attacks gain higher damage multipliers, giving the illusion that his Attack stat is raising.
